Most traffic control rooms are still driven by fixed-time plans and inductive loops buried in the road.
Those loops were laid decades ago, they fail quietly, and a junction running on a plan written for traffic that no longer exists looks identical on the wall to one that is working. The room gets rebuilt long before anybody audits what it is actually controlling.
